Patrik Sundberg 9ac969f0a0 fix(coding-agent): invalidate transcript cache when session file shrinks

Compaction and tool-output pruning can rewrite the session JSONL in
place, shrinking the file. When stat.size < bytesRead, the cache is
now invalidated and the file is re-read from scratch, preventing the
observer from getting stuck on a stale transcript.
