can1357 8fab9fb600 refactor(tools): restructured text result building to eliminate duplication

- Extracted in-memory text result building logic into reusable #buildInMemoryTextResult method to eliminate duplication between file and resource reading paths.
- Generalized result limit handling via ignoreResultLimits parameter to support skill resources without code duplication.
