djdembeck 821e6f39cf fix: set exclusive concurrency on exit_plan_mode tool

Ensures exit_plan_mode waits for all previous tools to complete
before checking if the plan file exists. Prevents race condition
when LLM calls exit_plan_mode before write completes.
