can1357 07cb2cecdf fix(coding-agent/modes): used latest session entries for /todo phase reads

- Added a private #currentPhases helper that reads todo phases from the latest session entries.
- Updated /todo handlers to use #currentPhases so existing todo states are loaded from entries when available.
- Added a fallback to session.getTodoPhases when no entry-derived phases exist.
