Replaced the controller-side switchActiveModel flag with a currentContextTokens hint on AgentSession.setModel, so the over-context decision is computed against the refreshed candidate metadata. setModel returns whether the live switch happened, and the Alt+M default-role path uses that to gate the live side effects.
- Added logic to `assembleYieldResult` to automatically accumulate incremental yields into arrays for schema-identified array properties.
- Updated `YieldTool` to bypass schema validation for incremental stream yields, allowing partial data emissions that don't satisfy the full output schema yet.

Address PR review: switchSession (/resume, /move, ACP fork/load,
RPC switch_session, extension switchSession) mutates the loaded
session file in place under the same AgentSession ref, so a WeakMap
keyed only on the AgentSession ref carried the previous
conversation's meter into the resumed one — the footer kept showing
the previous total after resuming a different idle session.
Snapshot the loaded sessionFile path in the per-session meter and
detect a real-to-real transition inside #meter(): on a swap, drop
the old meter and start a fresh one. The undefined → real first-save
transition only refreshes the snapshot (same conversation, same
identity, accumulated time preserved). #closeStaleActiveWindow now
routes through #meter() so the file-change check applies there too.
Adds two regression tests covering the real-to-real swap and the
first-save no-reset.
Address PR review: collab guest reconciles a state frame with
isStreaming === false by stopping the loader when the host's
agent_end never reached us (typically across a reconnect). With the
new active-time meter, the same path also has to call markActivityEnd
or the per-session window stays open and time_spent ticks forever.
Extract the close logic as reconcileGuestIdleHostState so a unit test
can exercise it directly without bringing up the full host/relay
welcome train.
Address PR review: SessionFocusController synthesizes agent_start on
mid-turn attach but does not pair it with a synthetic agent_end on
unfocus. With a single shared StatusLineComponent meter, returning to
the main session while a subagent was still streaming left
#activeStartedAt open, so the main status line kept ticking through
idle time after the subagent finished.
Replace the single #activeMs / #activeStartedAt fields with a WeakMap
keyed on AgentSession. markActivityStart / markActivityEnd /
getActiveMs / resetActiveTime all operate on the currently-attached
session's meter, so detaching from a subagent never bleeds its open
window into main. setSession closes a stale window (in-flight + new
session not streaming) on re-focus so a subagent that finished while
we were detached does not credit the detached gap.
Adds two regression tests covering both cases.
The time_spent segment rendered Date.now() - sessionStartTime, so an
idle session displayed hours of "time spent" while the agent did
nothing — the only inputs were wall-clock and the unmoving session
start.
Replace sessionStartTime with activeMs in SegmentContext and accumulate
inside StatusLineComponent across agent_start -> agent_end windows.
markActivityStart/markActivityEnd are idempotent (reentrant agent_start
events and superseded agent_end events never double-count); the segment
ticks live during an open window and freezes when the agent yields.
The session-boundary hook drops the now-meaningless wall-clock argument
and is renamed setSessionStartTime -> resetActiveTime; it zeroes the
accumulator and drops any in-flight window so /clear / fresh-session /
joined-collab paths start the meter at zero.

Codex discovery surfaced every `~/.codex/hooks/*.{ts,js}` file as an OMP
hook (silently defaulting untyped names to `pre:<basename>`), and
`discoverExtensionPaths` then handed those paths to `loadExtension` for
dynamic import. A standalone Codex hook script with a top-level
`process.exit(0)` terminated the host CLI cleanly — `try/catch` around
`await import()` cannot intercept a synchronous exit, so OMP died at the
`loadExtensions:start` startup marker with no error surface.
Two-layer fix:
- `packages/coding-agent/src/extensibility/utils.ts`: new
`withExitGuard` helper patches `process.exit` for the duration of a
guarded callback so an exit raises `ExtensionExitError` instead of
terminating the process; nested and concurrent guards restore correctly
via depth counter.
- `extensibility/extensions/loader.ts`, `extensibility/hooks/loader.ts`,
and `extensibility/plugins/manager.ts` wrap their dynamic-import sites
in `withExitGuard` so the existing per-module `try/catch` records the
intercepted exit as a load error and OMP keeps starting.
- `discovery/codex.ts:loadHooks` no longer treats arbitrary files as
OMP hooks: only `pre-<tool>.{ts,js}` and `post-<tool>.{ts,js}` are
registered. Files like `memory-bank-reminder.ts` are silently skipped
rather than imported as extension factories.
Adds regression coverage:
- `test/extension-loader-process-exit.test.ts` — `loadExtensions` /
`loadHooks` return errors and leave `process.exit` restored when a
module exits at import time; sibling modules still load.
- `test/discovery/codex-hooks-discovery.test.ts` — codex provider
registers `pre-*` / `post-*` files and drops everything else.

Adapted the existing high-non-ASCII idle-compaction regression to the new auto-downgrade contract: compact() is now expected to run, action ends as context-full, and the downgrade warning notice is asserted. Start event keeps reporting snapcompact since preflight runs inside the controller-installed try block.
