RegisteredToolAdapter previously defined renderCall/renderResult as
prototype methods that unconditionally delegated to the underlying
definition. When the definition had no renderers, these methods still
existed on the instance, causing ToolExecutionComponent to enter the
custom-renderer path, receive undefined, and silently discard tool
result text — extensions without renderers showed blank output.
Move render methods to instance properties assigned only when the
definition actually provides them, so the absence check in
ToolExecutionComponent works correctly.

Hashline prefixes (LINE:HASH|content) in read/grep output exist solely to
support the hashline edit mode. Agents that don't have the edit tool
(e.g. explore, plan, reviewer) gain nothing from them — they just waste
tokens and add noise.
resolveFileDisplayMode now takes a session-like object with an optional
hasEditTool flag. When false, hashlines are suppressed regardless of
settings. ToolSession gains hasEditTool (set from toolNames in the SDK),
and AgentSession exposes it as a getter over the tool registry.

Previously, /mcp enable|disable only worked for servers defined directly
in user or project config files. Discovered servers from third-party
configs (e.g. capability-provided) could not be toggled off without
removing them at the source.
This adds a disabledServers list to the user-level .mcp.json config that
acts as an overlay. When loading MCP configs, servers whose names appear
in this list are excluded alongside those with enabled: false.

- Add disabledServers field to MCPConfigFile type
- Add readDisabledServers/setServerDisabled helpers in config-writer
- Filter discovered servers against the disabled list during config load
- Handle enable/disable toggle for discovered servers in the MCP
