- Redesigned hashline patch syntax from anchor-based (`A-B:`) to hunk-header format (`@@ A..B @@`) with unified-diff compatibility.
- Removed `autoDropPureInsertDuplicates` option and simplified apply behavior to preserve duplicated boundary and context lines.
- Changed repeat operator from `^A-B` to `&A..B` and range separator from `-` to `..` for consistency with hunk-header syntax.
- Added image resizing and dimension notes to eval tool output; improved write tool hashline header sanitation for legacy formats.
- Removed 521 lines of boundary-duplicate absorption code and simplified parser to auto-convert bare body rows and unified-diff contamination.
- Replaced 4-hex content-derived file hashes with 3-hex opaque tags minted by InMemorySnapshotStore, making tags session-bound pointers rather than content fingerprints.
- Removed lru-cache dependency; replaced LRU-bounded per-path rings with a flat 4096-slot global ring using a scrambled permutation to prevent LLM tag extrapolation.
- Made SnapshotStore required in Patcher (was optional); tag resolution now drives stale-anchor detection instead of recomputing hashes at apply time.
- Changed literal payload sigil from `|` to `+` and accepted `^A` shorthand for `^A-A`; added lenient recovery for bare bodies, lone `-` rows, and overlapping bare/concrete block pairs.
- Replaced `LINE↑`/`LINE↓`/`A-B:` op sigils with unified `A-B:` anchor + `|`/`↑`/`↓` payload sigils.
- Added `mode: "replacement"` tag to insert edits so the applier distinguishes replace-bucket from insert-bucket lines.
- Removed lenient fallbacks (implicit continuation, inline payload acceptance, escaped delimiter stripping).
- Updated grammar, prompt, tokenizer, parser, applier, and messages to match the new format.
- Removed `path` field from hashline input parameters and function signatures across diff, execute, and params modules.
- Updated HashlinePatch.parse() calls to omit the path option, relying on the ¶PATH#HASH header in input instead.
- Removed unused warning tracking for escaped payload delimiters in hashline parser.
- Added detection and stripping of extra backslashes before indented payload rows, which models often emit when JSON-escaping the delimiter.
- Added `_input` field alias support in hashlineEditParamsSchema to accept provider-emitted variants.
- Added warning message for escaped payload delimiter acceptance to guide users toward canonical syntax.
- Added `parsePatchStreaming` and `Executor.endStreaming` to handle in-flight ops gracefully during incremental parsing, dropping pending ops with no payload to avoid phantom edits.
- Introduced `PatchSection.applyPartialTo` as a streaming-aware counterpart to `applyTo`, using the new streaming parser for diff preview generation.
- Removed the custom `buildHashlineNaturalOrderPreviews` function and replaced it with calls to `applyPartialTo`, centralizing streaming logic in the parser layer.
- Removed the `!` delete sigil and all associated parsing, validation, and tokenization logic. The delete operation is no longer a supported edit kind.
- Updated grammar, format constants, and error messages to reference only insert and replace operations.
- Simplified the executor's overlap validation to handle only replace operations.
- Reduced default context lines from 4 to 2 in diff generation.
- Removed leading and trailing ellipsis placeholders from generated diff strings.
- Clarified the destructive nature of the replace operator and added examples in the prompt.

- Returned tool results containing only header, preview, and warnings content blocks.
- Preflighted write policies for all sections before any commit in multi-section batches.
- Rejected duplicate canonical targets (e.g., `a.ts` and `./a.ts`) before writes begin.
- Fixed `after_anchor` normalization mutating cached edits across repeated patch applications.
- Fixed `detectLineEnding` to use first-occurrence style instead of majority vote.
- Added a dedicated @oh-my-pi/hashline package with parser, patcher, filesystem, snapshots, and release metadata.
- Migrated coding-agent hashline and stream entrypoints to @oh-my-pi/hashline and removed old hashline module exports.
- Changed multi-section hashline execution to validate section hashes and flush diagnostics only at the final commit.
- Added session fileSnapshotStore support and rewired edit/read/search/write tools to use it instead of fileReadCache.
- Changed multiline payload syntax so continuation lines must start with `+`; that prefix is stripped before writing.
- Raw unprefixed lines after an op now throw an error instead of being silently accepted as payload.
- Removed `PAYLOAD_LINE_PREFIX_DEMOTED_WARNING` and the nested-replace demotion path; inner `N:` ops inside a pending `A-B:` now raise an overlap error.
- Raw blank lines between ops are ignored; use `+` alone for an empty payload line.
- Added `ToolTier`, `ToolApproval`, and `ToolApprovalDecision` types and exported approval APIs.
- Updated approval-mode options from `auto|prompt|custom` to `always-ask|write|yolo` and defaulted mode to `yolo`.
- Changed approval resolution to apply per-tool decisions first, then mode-tier limits, with legacy-mode migration.
- Assigned read/write/exec `approval` and approval-detail prompts across built-in, custom, extension, and MCP tools.
- Adjusted hashline natural-order preview handling so op-insert and op-replace tokens now emit inline body content as payload when available.

- Updated documentation and test fixtures to reflect new hashline syntax with file hashes, colon separators, and delete operator throughout.
- Removed vim edit mode and automatically map existing vim configurations to hashline mode.
- Deleted VimTool class, VimEngine implementation, and all vim-specific editing logic (2409 lines).
- Removed vim mode from EditMode union type, edit tool strategies, and configuration schemas.
- Deleted vim parser, command handler, buffer manager, and renderer modules.
- Updated documentation and tests to remove vim mode references and add deprecation mapping.
- browser tool's existing-tab re-nav defaults to waitUntil: 'load' (matching
new-tab path); identical acquireTab() calls no longer hang on dev servers
- patch tool error path uses caller-supplied relative path; absolute
resolvedPath stays in structured context only ($HOME no longer leaks to TUI)
- splitInternalUrlSel keeps mcp:// resource URIs opaque even when they end in
':raw' or '/:1-50' (McpProtocolHandler matches by verbatim URI)
- find tool: timeout signal honored by onMatch; partial results sorted by
mtime desc; backslash-escaped commas skipped in path-list validation
- DAP throwPreferredDapStartError waits up to 50ms for the underlying
launch/attach error instead of one microtask

- Changed hashline format to canonical `§` section headers and `"`/`"`/`≔` operations across grammar, parser, and docs.
- Reworked range and op parsing so single anchors are valid, legacy `-`/`-=` ops error, and empty `≔` payloads now delete ranges.

The size+mtime check from 094273df5 is unreliable on filesystems with
coarse mtime resolution: a same-length rewrite within the same tick
(e.g. "a" → "b") leaves both fields unchanged and falsely trips the
"file content did not change on disk" guard. CI on ubuntu Bun 1.3.14
hit this in the create-then-update aggregation test.
Re-read the file post-write and compare bytes to the previous content
instead — deterministic regardless of FS timestamp granularity.
executePatchSingle returned success based on the writethrough callback
resolving, but the LSP-backed writethrough could resolve to an in-memory
editor buffer while disk stayed unchanged. Capture pre-write mtime+size,
re-stat post-write, and throw a ToolError when nothing changed.

- Added a regression test in edit renderer tests confirming both canonical "@@" and longer "@" runs render as clean file paths without extra "@" characters in titles.
Addresses the codex review comments on #1015 plus a sweep of adjacent
ACP conformance gaps surfaced while wiring them up.
Tool call + diff metadata
- acp-event-mapper: thread session cwd through and resolve every
`ToolCallLocation` (initial args, in-flight updates, result details)
to absolute paths against it; ACP requires absolute paths for
client-side file mapping.
- edit/modes/patch: emit the destination path for moves in the diff
result so post-edit "open file" actions land on the new file.
Permissions
- agent-session: pass cwd into `extractPermissionLocations` and resolve
raw `path`/`file`/etc. fields against it before sending
`session/request_permission`.
- agent-session: gate the permission wrapper on
`bridge.capabilities.requestPermission && bridge.requestPermission`,
matching the read/write/bash capability+method pattern.
acp-agent
- `authenticate`: validate `methodId` against the methods advertised by
`initialize` and reject anything else, so malformed clients fail fast.
- `setSessionConfigOption(MODE_CONFIG_ID)`: also emit
`current_mode_update` so clients tracking `modes.currentModeId` see
the same transition `session/set_mode` would produce.
- Pass `runtime.notifyConfigChanged` to builtins; emit
`available_commands_update` from a shared `reloadPlugins` helper
reused by `/reload-plugins`, `/marketplace`, and `/plugins`.
- prompt resource handling: route `resource` content with `image/*`
MIME into the `images` array instead of dropping it as an opaque
blob; non-image blobs still fall back to the URI placeholder.
- pass session cwd to the event mapper.
Builtins
- model: call `runtime.notifyConfigChanged()` after a successful
`setModel` so the ACP config selector reflects the new model
immediately.
- mcp: redact query strings and userinfo from MCP server URLs before
emitting them in `/mcp list` (prevents leaking `?exaApiKey=…` style
secrets); wire `manager.setAuthStorage(...)` before `prepareConfig`
in `/mcp test|resources|prompts` so OAuth servers can refresh tokens.
- ssh: reject non-integer `--port` values via a `^\d+$` guard instead
of silently coercing through `Number.parseInt`; list project hosts
first and dedupe user-scope duplicates to match capability-loader
precedence.
- export: reject clipboard aliases (`--copy`, `clipboard`, `copy`)
before passing them to `exportToHtml` as a filename.
- compact / force / move / browser: surface underlying failures via
`usage(errorMessage(...))` instead of letting them crash the command.
- session save|delete: route through the active SessionManager so the
persist writer is consulted and stale storage references are removed.
- marketplace / plugins / reload-plugins: call `runtime.reloadPlugins()`
on install/uninstall/upgrade and enable/disable so slash command
registries and command lists refresh consistently.
- shared.usage: make async and `await runtime.output(...)` so
`sessionUpdate` text is never dropped or reordered.
- types: document the new `reloadPlugins` and `notifyConfigChanged`
runtime hooks.
bash tool
- Use a shared `fireKill()` from the abort listener so `session/cancel`
terminates the remote command immediately instead of waiting for the
next `currentOutput()` round trip.
- Race `currentOutput()` against the abort signal so a stuck
`terminal/output` RPC cannot delay cancellation.
- Kill the terminal before reading final output on timeout so a slow
output read cannot let a timed-out command keep running past the
enforced timeout.
Tests
- acp-agent.test: extend the existing config-option assertions to
verify both `model` and `thinking_level` changes emit
`config_option_update` notifications scoped to the right session.
- acp-builtins.test: cover `/model` emitting both
`notifyTitleChanged` and `notifyConfigChanged`; lock in the parsed
`mcp add` / `ssh add` call shapes so future arg-parser regressions
fail the test instead of silently writing different configs; add a
`reloadPlugins` stub plus a typed `notifyConfigChanged` slot to the
shared test runtime factory.
- acp-stdout-hygiene.test: drain stderr in parallel and assert no
JSON-RPC frame leaks onto it; terminate the spawned process so the
stderr pump resolves deterministically.
CHANGELOG: itemize the above under `[Unreleased] > Fixed`.
CI
- bun run check: clean (TS + Rust)
- bun run test: 4128 pass / 689 skip / 0 fail (TS); 252 pass / 0 fail
(Rust nextest)
- bun run ci:test:smoke: --version / --help / `stats --help` all OK
