Adds glob pattern support for skill filtering:
- --skills <patterns> CLI flag (comma-separated glob patterns)
- includeSkills setting in settings.json
- ignoredSkills now supports glob patterns
- ignoredSkills takes precedence over includeSkills and --skills

- Remove {baseDir} placeholder (use relative paths)
