- Prohibited narration of token limits, budgets, effort estimates, and scope-inflation speculation during task handling.
The 30 second default for 'ask.timeout' silently auto-selects the
recommended option mid-deliberation, which surprises users who are
weighing options the agent flagged as having materially different
tradeoffs. The setting was meant to be opt-in: 0 already means
'wait indefinitely' and plan mode already forces the timer off.
Flip the default to 0 so a fresh install matches the documented
'wait until the user replies' behavior, and update the AskTool
prompt so the model expects unlimited reply time by default. Users
who liked the auto-select can still set a positive timeout from
the Interaction settings tab.
Add a regression test asserting the dialog timeout is not passed
to the underlying selector when 'ask.timeout' is unset.
- Replaced project, subagent, and system prompt templates to use bracket-style `[TAG]...[/TAG]` markers instead of `<|START_*|>`/`<|END_*|>` markers.
- Updated system-prompt and subagent reminder tests to assert the new bracket tag format for role, context, and contract blocks.
- Added an Unreleased changelog entry for the subagent batch registry initialization behavior.
- Replaced `===== ... =====` eval cell headers with `*** Begin ` / `*** End ` markers; legacy format remains renderable in HTML exports.
- Replaced hashline patch grammar with `*** Begin Patch` / `*** End Patch` envelope; old inputs without the envelope are still accepted.
- Extracted `sniffEvalLanguage` into a shared `sniff.ts` module reused by the parser and tool.
- Added `docs/ERRATA-GPT5-HARMONY.md` and `scripts/session-stats/harmony_backtest.py` documenting and backtesting the GPT-5 Harmony-header leak defect.
- Deleted `bash-normalize.ts` and its tests; output truncation is handled by the streaming tail buffer and artifact spillover.
- Removed `head`/`tail` schema fields from `bashSchema` and `BashToolInput`.
- Updated system prompt and bash tool prompt to forbid `| head`/`| tail` pipes and other anti-patterns, directing the agent to use dedicated tools instead.
- Added explicit prompt markers and wrappers across system templates, including `[env]`, `[role]`, `[coop]`, `[closure]`, and `[now]`.
- Removed `renderTemplate` and `sectionSeparator` flows, deleted `task/template.ts`, and switched to per-task `renderSubagentUserPrompt` rendering.
- Updated system prompt assembly to `shortenPath`-normalize `cwd`, append rendered now metadata, and preserve trailing `[now]` blocks.
- Removed legacy template tests and added prompt-composition tests for ordered `[contract]`->`[project]`->`[now]` blocks and context-only system placement.

- Added guidance warning not to renumber line references when stacking multiple patch operations.
- Updated the hashline grammar to require a full `LID .. LID` range for block operations.
- Enhanced `parseRange` to reject single-anchor syntax and malformed ranges, and to require duplicated anchors for one-line edits.
- Reworked hashline prompt examples/tests accordingly and added coverage for single-anchor delete/replace forms.
- Clarified the hashline guidance to select a self-contained syntactic unit before narrowing the edit operation.
- Added a multiline destructuring/call example showing the full construct replacement pattern and safer alternatives.
- Updated anti-pattern instructions to reject partial-boundary replacement payloads and continuation-fragment edits.
When a patch introduces a new type that crosses a function/module
boundary (event, message, RPC frame, enum variant, etc.), the reviewer
must locate the dispatch point on the consuming side and confirm the
new type is handled. This class of bug is invisible from the diff alone
because the silent drop lives in untouched routing code.
Motivated by a missed P2 in PR #987: a new open_url extension_ui_request
was emitted by login() but RpcClient#handleLine had no case for it and
silently dropped every frame, leaving callers with no auth URL and the
command hanging until timeout.
- Updated `hashline.md` to document brace-boundary edit patterns for block replacement, signature-only edits, and safe interior insertions.
- Added common-failure guidance on range boundaries, duplication checks, truncated anchors, and preferring narrower ops over wide replacements.
- Adjusted the anti-pattern example to match the corrected replacement range behavior.

- Updated eval tool guidance to show Python `asyncio.run(...)` usage.
- Added an instruction in the Python eval prompt explaining that notebook cells run in an IPython kernel with a live event loop.
- Documented that users should use top-level `await` directly and avoid `asyncio.run(...)` due to the running event loop.
- Kept the existing failure-handling guidance unchanged while clarifying async execution behavior for Python cells.
- Updated system prompts to require every active turn to end with a tool call and clarified that reminders should default to resuming work unless the task was complete or genuinely blocked.
- Reworked the yield-reminder text to disallow fake blocker reasons and to permit continued tool-calling instead of forced immediate yields.
- In `runSubprocess`, applied `toolChoice` only on the final yield retry by adding an `isFinalRetry` check before sending the reminder.
- Updated the read tool docs to define URL selectors as `:50`, `:50-100`, and `:50+150` and to document the `https://host/:port` form for URLs with explicit ports.
- Changed URL selector parsing to use the file-style numeric range format, treated `raw` as a separate token, and updated the invalid zero-line message to direct users to `:1`.
- Refined embedded URL selector extraction to validate the base URL first and then match selectors against the new numeric range regex.
- Removed the read CLI argument and tool schema field so read requests no longer accept custom timeouts.
- Updated URL read handling to stop forwarding timeout values and execute URL reads without a timeout parameter.
- Standardized URL read fetching to a fixed 30-second timeout and dropped timeout metadata from URL call rendering.
- Removed [blocked]-focused instructions from failure handling and pre-yield checks in the system prompt.

- Imported and rendered the prompt in `src/task/commands.ts` as `orchestrateMd`.
- Appended the new prompt to `EMBEDDED_COMMANDS` so it is registered with existing embedded command templates.
- Updated the hashline prompt to require replacement ranges to cover entire statements or expressions, preventing broken syntax when applying changes.
- This clarified how replacement anchors should be chosen for function calls, literals, blocks, and control-flow branches.
- Added a new completion-honesty section to the coding-agent system prompt with stricter delivery constraints.
- The prompt now requires all acceptance criteria to be met before completion, prohibits scope reduction without approval, and forbids delivering stubs, placeholders, or fake implementations as finished work.
- Verification language was tightened so completion claims must reflect actual verification and not be inflated by partial checks.
- Added `.ipynb` detection and editable-cell conversion utilities, including merge/serialize helpers in `edit/notebook`.

- Removed persona preambles ("You are an expert...") in favor of direct imperatives.
- Stripped redundant MUST/SHOULD modals where plain prose suffices.
- Condensed multi-sentence instructions into tighter single-line equivalents.
- Updated the `init` operation to require a `list: [{phase, items: string[]}]` payload and to replace any existing list.
- Updated the `append` operation description to state tasks are appended to a given `phase` and that the phase is lazily created.
The internal class is now SearchToolsTool but the wire name stays
"search_tool_bm25" so persisted MCP selections in user session files
continue to resolve correctly.
The BM25 corpus is now drawn from the generic DiscoverableTool list
(built-ins, MCP, extension, custom) rather than only MCP tools.
createIf fires when either tools.discoveryMode !== "off" or the
legacy mcp.discoveryMode is true.
AgentSession gains generic discovery methods (isToolDiscoveryEnabled,
getDiscoverableTools, getDiscoverableToolSearchIndex,
getSelectedDiscoveredToolNames, activateDiscoveredTools). The existing
MCP-named methods are kept as thin shims that filter by
source === "mcp" so existing callers continue to work.
Updated the tool's prompt copy to advertise discovery across all
sources rather than MCP only. Tests extended for the new shape.
